To use this option, the person receiving food must complete the recipient information section on the By-Proxy form, and the person shopping on their behalf will complete the proxy information section. Please bring the completed form when visiting the pantry.
By-proxy shopping is intended for neighbors who are unable to come to the pantry themselves. To help us serve everyone efficiently, we may limit the number of households one person can shop for at a time based on the length of the line and pantry capacity. A new form is needed each time.
